RELEASE FORMS

In order to complete a sale, it is your responsiblity as a production to have all proper release forms signed, and confirm that all footage is your ownership or have permission to use commercially. 

Please complete all forms below for each production. It is not necessary to fill out a new form for a new edit or cutdown. Failure to complete forms may lead to a delay in payment.
